WHAT IS INFERTILITY? WHO SHOULD APPLY?

 

Infertility is the inability of a couple to achieve pregnancy despite unprotected and regular sexual intercourse for 12 months. However, those over the age of 35 or those with a family history of early menopause, ovarian cysts or surgery can apply if pregnancy has not occurred for 6 months.

 

WHAT ARE THE TREATMENT STEPS?

 

After a detailed examination of the couple, necessary hormonal tests, sperm test, HSG (hysterosalpingography-uterus film) and tube evaluation, in some cases chromosomal genetic tests, and additional tests specific to the couple may be performed.

After the evaluation, it is determined whether there is a chance of spontaneous pregnancy and the natural follow-up waiting period. If there is no chance of pregnancy or very low or if pregnancy does not occur in the expected period, treatments that increase the chance of fertilization are started. The first stage is ovulation regulation and stimulation and intercourse planning. If it is not successful, the next level treatment is taken and insemination can be performed. Laparoscopic hysteroscopic operations may be required to solve problems such as myoma, polyp, cysts, intrauterine septum adhesion, hydrosalpinx that cause infertility in the uterine tubes or ovaries or that may negatively affect the treatment. In the last stage, in vitro fertilization is performed.   Genetic screenings with PGD (genetic research of the embryo), male infertility research TES (surgical sperm extraction from the testicles) may be required. The stages to be passed and the time period for each couple are determined by the treatment program specific to the couple’s condition.

 

WHAT IS IVF?

 

In vitro fertilization is the final stage in infertility treatment. In vitro fertilization consists of IVF (in vitro fertilization) and ICSI (Intracytoplasmic Sperm Injection) techniques. The treatment involves bringing together the egg taken from the woman and the sperm taken from the man in a laboratory environment and transferring the embryos formed and developed after fertilization into the uterus. Infertility problems are observed in 15% of couples of reproductive age and in 20% of these couples, in vitro fertilization is required.

 

WHO CAN RECEIVE IVF TREATMENT?

IVF or microinjection;

  1. Tubes are blocked or abnormal,
  2. In the presence of ovulation disorders and reduced ovarian reserve,
  3. Lack of sperm in the male or lack of sperm with sufficient movement and morphology
  4. Cases that do not respond to other treatments,
  5. The presence of genetically transmitted diseases,
  6. Polycystic ovary syndrome
  7. It is applied in cases of unexplained infertility.

WHAT ARE THE STAGES OF THE IVF PROCESS?

 

After the spouses’ general health and infection screening tests, the treatment protocol is determined by evaluating the woman’s menstrual period for 2 or 3 days.

Women develop one egg per month during each menstrual period. The first step of treatment is Controlled Ovarian Hyperstimulation (COH) using gonadotropin hormone drugs to stimulate the ovaries.

The aim is to obtain a large number of high quality eggs in a menstrual cycle. The development of the follicle, which is the sac in which the egg is located, is measured and controlled with ultrasound examinations performed at certain intervals. When at least 3 follicles reach 17-18 mm, the trigger needle HCG (Human Chorionic Gonadotropin or R-LH) injection is performed. At this time, the endometrium thickness should also be at least 7 mm. 34-36 hours after the trigger needle, our second stage, egg collection, is performed under anesthesia and with ultrasound guidance.

After the egg collection procedure, treatments are started to maintain hormonal balance (Luteal phase support). The eggs taken to the embryology laboratory are prepared for the procedure.

On the day of the oocyte collection procedure, the man also gives a sperm sample. The most ideal method for sperm collection is masturbation. However, if there is no live sperm in the semen, sperm is surgically taken from the testicles.   With the ICSI method, a sperm is injected into an egg.   The development of the obtained embryos is monitored daily under appropriate laboratory conditions .

The highest quality embryo or embryos with the highest potential for pregnancy   are selected for transfer on the 3rd or 5th day of their development.

In the final stage, the selected embryo is transferred into the uterus with a soft catheter under ultrasound guidance. The remaining good quality embryos can be frozen and stored for later use. Bed rest is applied for 30 minutes after the transfer. It is known that longer rest does not affect the results. A pregnancy test, beta-hCG, is measured in the blood 10 days after the 5-day embryo transfer and 12 days after the 3-day transfer. As a result, in IVF treatment, your egg and your partner’s sperm are fertilized in a laboratory environment and the baby carrying your genes is placed in your uterus
