Prominent ears are typically the result of a congenital or developmental condition where the ears appear overly large, protrude outward, and lack the natural folds that define their shape. This condition can be effectively addressed through otoplasty, a surgical procedure that repositions the ears closer to the head and recreates the missing folds to restore a more natural look. Otoplasty improves facial symmetry and proportion, offering a more balanced appearance. Even subtle ear deformities, when corrected, can have a positive impact on self-esteem and overall facial harmony.

The ideal time for otoplasty is during the preschool years, around age 5, when ear growth is nearly complete. Performing the procedure at this stage can help prevent potential psychological distress associated with prominent ears. That said, adults can also benefit from the surgery if they wish to correct the appearance of their ears.

Otoplasty is carried out under proper surgical conditions and anesthesia. For both children and adults, the procedure can be performed under sedation, allowing a restful, sleep-like state without full general anesthesia. An incision is made discreetly behind the ear, through which permanent sutures are used to reshape and reposition the ear to the desired angle. The resulting scar is well hidden within the natural crease behind the ear and remains virtually invisible, even when the hair is tied back.

As part of preoperative care, patients should stop smoking at least four weeks before the surgery and avoid aspirin, herbal teas, and dietary supplements for at least 10 days prior.

Post-surgery, a large dressing and small drains are applied to support healing, with drains typically removed the following day and the dressing gradually reduced. Some redness and swelling are common once the dressing is removed and should not cause concern. After this point, patients are advised to wear a soft headband during the day for two weeks and at night for six weeks to keep the ears in position and protect the surgical results, especially while sleeping.

Otoplasty is generally not painful. Thanks to the local anesthesia used during the operation, discomfort is minimal, and mild pain relievers are usually sufficient for the short recovery period. Most swelling resolves, and the ears settle into their final shape within six weeks.

As a cosmetic procedure that alters the ears’ position, shape, and size, otoplasty should always be performed by a qualified plastic surgeon.
